Direct Cuspal Onlay/Overlay

This patient attended with a temporary filling LL5 and large failing old composite restorations on endodonitically treated LL6 and LL7. She did not wish indirect treatment options to Crown or Onlay the molars as these would be destructive on already heavily restored teeth. She wishes a direct more minimla treatment alternative that would provide the same protection and strength as an indirect option.

The teeth were isolated with a rubber dam, disclosed, and pre-wedged with Diamond Wedges. LL5 preparation included Selective Caries Removal, occlusal beveling, and lateral wall beveling, while LL6 involved cuspal reduction and chamfering.

Biofilm was removed with aluminum trihydroxide powder and Aquasol (Aquacare). Evolve matrices were selected with the Evolve Height matrix Indicator, placed with a Diamond Wedge and Twin Ring, and the LL7 distal matrix was spot-welded. All seals were confirmed before restoration.

The 2-Point Spot Weld technique was applied to the mesial matrix on LL7 to ensure proper contact point formation. Bioclear Injection Molding was performed using 3M™ Scotchbond™ Universal Plus Adhesive, warmed 3M™ Filtek™ Bulk Fill Flowable Restorative, and 3M™ Filtek™ One Bulk Fill Restorative composite in shade A3. After removing the Twin Ring and Evolve matrices from LL7, the Twin Ring was re-seated on LL5, and the 2-Point Spot Weld and injection moulding process was repeated.
Both teeth were shaped simultaneously using a carbide bur and 3M™ Sof-Lex™ coarse discs.

The restoration was polished using Bioclear Rock Star Polish, Bioclear Magic Mix, and SS White Jazz Polishers, achieving a smooth, consistent shine.

Final photos reveal anatomical Class II and Overlay restorations with broad contact areas and strong monolithic composite, ensuring durability, healthy periodontal tissues, and excellent polish retention.

The patient returned two years later requesting a Bioclear Overlay for LL6.

The tooth was isolated with a rubber dam, disclosed, and pre-wedged with Diamond Wedges. Preparation involved cuspal reduction and chamfering. Biofilm was removed and Evolve matrices were placed with a Diamond Wedge and Twin Ring.

The 2-Point Spot Weld technique was applied to the distal matrix, followed by Injection Moulding for the distal portion of the restoration.

After removing the Twin Ring and Evolve matrix from the distal LL6, the process was repeated for the mesial portion. The tooth was then shaped with a carbide bur and 3M™ Sof-Lex™ coarse discs.
